Aftercare Relapse Prevention (at Solutions Family Resource Center)
Aftercare Relapse Prevention
is a 6-month program which offers comprehensive case management to
assist clients in establishing an identity within their community. This is accomplished by assisting clients with meeting goals to achieve self-sufficiency.
Weekly
participation in individual coaching sessions, group sessions, and
community 12-Step meetings reduce relapse by supporting clients in
achieving social re-entry goals such as gainful employment, navigating
social and government systems, family reunification, and obtaining
affordable stable housing.
Aftercare consists of two tracks:
Track One - Unemployed
Clients
participating in this track receive support in preparing and actively
participating in job search activities such as enrolling in school or
career training programs to increase employment marketability and
combined job search and career enhancement. Continued case management combined with weekly alumni support groups helps clients meet their goal of gaining employment.

Employment Preparation |
Track Two - Employed/Underemployed
Clients
who are employed receive support with improving their interpersonal
skills as is necessary for job retention, while underemployed clients
receive support in upgrading their employment status. Continued
case management combined with weekly alumni support groups helps
clients meet their goal of upgrading and retaining employment.
